Life with Us:

Transitional Care is a multidisciplinary team providing care for patients as they transition through their health journey.  We are a 35 bed inpatient unit, encompassing 29 Transition Care beds and 6 GEM restorative beds.   Our patients are cared for on the unit whilst awaiting residential care placement or in the process of making decisions related to long term care. Our patients are generally older adults and are recovering from an acute admission.

• Provide quality care to patients in collaboration with the health professional
• Assist patients in activities of daily living, such as personal care tasks, toileting and cooking
• Set up of treatment areas and position patients or equipment ready for treatment
• Assist in the provision of ongoing individual and group treatment programs

The role of the Allied Health Assistant is diverse.  You will be seeking an opportunity to take the next step with your career and be keen to demonstrate your previous experience and sound clinical skills.  Highly developed oral & written communication skills and be able to work within a multidisciplinary team environment and individually is essential. Completion of Mayfield Allied Health Course or equivalent is desirable.
