Social Worker (SW2)

  • Part Time Ongoing position for a Grade 2 Social Worker
  • Join our dedicated multidisciplinary team!
  • Based at The North West Community Care Unit in Broadmeadows

The North West Community Care Unit (NWCCU) provides a 24-hour community based residential rehabilitation program to people who experience long term serious mental illness and disability.


The CCU is a therapeutic environment in which rehabilitation programs are tailored to meet individual resident needs. The residents are people who require high levels of clinical support and intensive rehabilitation to support their mental health and daily living.


The CCU team work collaboratively with our Prevention and Recovery Centre (PARC) to support an integrated model of care.


Your new role:

  • Provide specific social work skills and mental health assessments for both North West PARC and NWCCU
  • Work within a multidisciplinary team, provide psychological assessments, intervention and psychotherapy to identified consumers
  • Work collaboratively with families/carers and community partners utilizing the recovery model of care
  • Plan and facilitate therapeutic groups, and also provide consultation to other members of the team
  • Work with the leadership group and allied health to review, implement and evaluate the NWCCU group program

About You:

  • Commitment to a strengths based approach, person centred recovery and thinking outside the box
  • Theoretical knowledge of social work interventions in mental health
  • Demonstrated evidence of professional development in a number of areas relevant to Demonstrated experience in Public Health and Community Case Management
  • practicing as a Social Worker in a mental health setting
  • Demonstrated skill in providing a range of social work assessments and individual interventions to the clients and their carers
