Physical Health Nurse

• We are seeking a Registered Psychiatric Nurse to join our Physical Health Program
• Part Time Position working 56 hours per fortnight
• Based in Moreland Community Care Team, Coburg

Your new team

• We a service working together to provides treatment and care coordination to people with a serious mental illness who live within the City of Hume and Moreland
• The catchment area has a high proportion of people from a non-English speaking background and a high concentration of people with a psychiatric disability
• You will help us promote therapeutic and enabling partnerships with consumers and their carers that promote mental health and physical health self-management, improvement in independent functioning and daily living skills, and improved treatment adherence
• Promote communication and collaboration between mental health clinicians, primary care health professionals, and community health and lifestyle program worker

About your new role:

• Work collaboratively with nursing, medical and allied health clinicians to identify consumers who require specialist physical health and metabolic monitoring.
• Make provision for consumers identified as having, or at risk of developing, physical co-morbidities receive comprehensive physical health assessment and metabolic monitoring, and that the findings inform targeted therapeutic intervention planning that is incorporated in the consumers Individual Service Plan.
• Liaise with the consumer’s treating medical officer to ensure requisite special tests and investigations are ordered, and a process is in place to ensure the prompt interpretation of results and subsequent treatment planning. Liaise with nursing, medical and allied health clinicians to identify consumers who have been identified as at risk, or are at risk of developing, physical comorbidity whose ISP include supported referral to a community general practitioner and other primary care services;
• In partnership with the consumer, their carer, their case manager, and their treating doctor, identify aspects the consumers planned care that can be delegated by the Registered Nurse to the Enrolled Nurse.

About you:

• Experience working as a mental health nurse within a multidisciplinary care team
• A sound understanding of the physical health problems associated with serious mental illness.
• Demonstrated knowledge, skills and experience, in the provision of comprehensive physical health assessment, metabolic monitoring, planning of therapeutic interventions, and evaluation of clinical outcomes
• Demonstrated provision of high quality, person-centred evidence-based care in the community mental health context.
