Colorectal Nurse Coordinator

  • Seeking an enthusiastic and self-motivated individual to join the Colorectal Team 
  • Full time fixed term -  76 hours per fortnight for first 6 months
  • Then Part time ongoing - 32 hours per fortnight

Your new role:

  • timely access to Specialist Clinic services and inpatient care
  • safe, effective, and patient centered care
  • access to digital health strategies such as telehealth and the Health hub to optimise care

For the first 6 months (76 hours per fortnight) you will work closely with the Specialist Clinics Reform Project Manager, responsible for developing, implementing, and sustaining strategies to improve the patient journey across the continuum of care, including out-of-hospital patient management and liaising with GP’s.

 

After which as the part time ongoing (32 hours per fortnight) Colorectal Nurse Coordinator (CRNC), you will:

  • Work in consultation and collaboration with the Colorectal Medical staff, Nurse Unit Managers, and Specialist Clinics team to assist in the coordination of care for patients with colon and rectal diseases.
  • Act as a resource person for the health service, promoting best practice in the provision of patient care through clinical initiatives and service innovation.
  • advocate for, and support, the patients under your care

About you:

  • Can demonstrate superior clinical expertise together with outstanding interpersonal negotiation and communication skills.
  • Recent acute medical and surgical nursing experience
  • Demonstrated ability to be innovative, resourceful and adaptive to change
  • Strive to make a difference in patient care.
  • lead by example with kindness and compassion

Selection Criteria Desirable: Experience in colorectal surgical nursing
Desirable: Experience in clinically based nursing education
Desirable: Experience in management, quality or clinical education.
Desirable: Postgraduate qualifications in a relevant area of specialty
