Advertisement text Entry Level Position Clinician Case Manager – EPPIC Clinical Stream, Continuing Care Team – Region A – Parkville site Orygen Youth Health – Clinical Program NorthWestern Mental Health Full Time Fixed Term Position until July 2017 Social Worker or Occupational Therapist Grade 1 About Us: ORYGEN Youth Health consists of a specialist mental health service for young people, integrated with an extensive research program, and with a focus on the translation of sound evidence into practice at local, state, national and international level. EPPIC (Early Psychosis Prevention and Intervention centre) is a major clinical stream within the Continuing Care Teams. Clients attending the EPPIC Clinical Stream are young people (15 -25 years) who are experiencing a first episode of psychosis. The Role: The ELP clinical case manager works within a specialist multidisciplinary team environment and is responsible for devising, implementing and evaluating an appropriate treatment plan for each young person on his/her caseload and their family. As an ELP position it is well supported in terms of supervision, training opportunities, and specialist consultation. The position involves provision of clinical assessments and psychological treatments, assertive outreach and case management tasks, such as liaison with other agencies and advocacy on behalf of the young person. The incumbent is expected to function autonomously, carry a limited caseload of clients, participate in clinical review meetings and practice according to the philosophy and protocols of the service. The service developments aim to increase local service delivery with stronger partnerships with local care providers, including headspace. In the new model of care, clinicians will have greater capacity to provide specialised evidence-based interventions for young people and their families by having smaller caseloads and access to enhanced professional development. Innovative and collaborative models of care will be developed with the primary care sector, including opportunities for the incumbent to pursue private practice within the headspace environment.
