Life With Us:

 

St Albans Community Care Unit

  • 10 Unit complex accommodating 20 beds, with 24 hour nursing care support. It provides recovery-focused rehabilitation in a community residential setting, to consumers who typically have multiple and complex needs
  • Aims to increase an individual’s psychosocial independence through a variety of person-centred programs to develop daily living skills
  • Provides access to multidisciplinary clinical support and treatment, including regular medical psychiatric review, Allied Health and 24 hour nursing support
  • Develops residential rehabilitation group programs based on need
  • Undertakes individualised assessment, care planning and review of suitability for less restrictive treatment and care
  • Provides psycho-education and support to family/carers and promotion of continued links between people and their family/carers
  • Facilitates the monitoring, engagement and support of people receiving compulsory treatment under the provisions of the Mental Health Act 2014
  • Promotes community links and partnerships to foster integration into the broader community

Learning and development opportunities: As part of the Multidisciplinary team at CCU, you’ll participate in regular individual and group supervision, have access to high quality in-house and external training, be able to contribute to the development and delivery of targeted social work interventions (individual and group-based) for consumers referred to the St Albans CCU.. Furthermore, working in a multidisciplinary team with psychiatrists, psychology, occupational therapists, nurses and you will further develop your holistic understanding of mental health and add significantly to the breadth of your skills.


Clinical challenges: You’ll be working with consumers from multicultural backgrounds with a broad range of low and high-prevalence psychiatric presentations and will be liaising with other service providers internally and externally to establish comprehensive and consistent best-practice responses.

Essential:

 

  • Experience in providing psychosocial interventions individual and family counselling, group work, advocacy and community development and education
  • Knowledge of Social Work and ability to translate findings into recommendations
  • Understanding of consumer engagement and multidisciplinary working
  • Skilled clinical practitioner with excellent interpersonal skills and sound I.T skills

Selection Criteria Essential: Minimum of 2 years’ experience post-graduation as a Social Worker
Essential: Ability to undertake a range of social work interventions, including individual, family, group work and community liaison to consumers with complex mental health and psychosocial problems, their families and carers, incorporating a broad range of the theoretical knowledge and perspectives, in part
Essential: Collaborate with members of the multi-disciplinary team in decision making pertaining to high quality standards of patient care. This includes assessment, planning, implementation and evaluation of care
