We are looking for a compassionate and open minded professional to join our Adult Community Service (Waratah Clinic Moonee Ponds), a service offering acute and non-acute work in both a clinic based, and outreach setting.

 

It is a senior position, however we are encouraging non-senior candidates wishing to work towards the senior role in their first 12 months of employment to apply!  Our service provides a range of acute, brief and longer term interventions to assist people with psychiatric disorders.  Consumers and their family/carers will have access to care in the community to meet their needs and preferences, provided by an integrated Community Team in the relevant catchment area.

 

A typical day in this position might include:

  • Psychiatric assessment of adults (16-65 years of age) in acute, sub-acute or non-acute settings
  • Underpinned by a recovery-oriented approach, the Community Team coordinates crisis responses, targeted, evidence-based intervention (brief & longer term), and consultative responses to primary health care providers.
  • Engaging with consumers, carers, CMHSS and other services to provide best practice for recovery, using a range of evidence-based approaches
  • Development and delivery of service improvements
  • Maintaining documentation and other data associated with your work

About you:

 

You enjoy working in a multidisciplinary team environment, proactively look for opportunities for self-development and improving service delivery, have an ability to problem-solve without being asked, and demonstrate behaviours reflective of Melbourne Health’s values.

 

You enjoy supporting a recovery approach to care, and promote a friendly, respectful and supportive environment within the Community Team and across the AMHS.

 

You are adaptable to change priorities to meet the needs of the consumer or team, adjust quickly to new tasks, be flexible and remain positive in a changing environment.

 

You communicate with well developed skills in writing and an ability to promptly prepare documentation and the ability to communicate effectively with consumers, families/carers, colleagues and other service providers.

 

You have: 

  • Experience - A RPN with at least 4 years experience in the assessment, delivery of treatment and provision of targeted interventions and support to consumers with complex mental health related needs and their families/carers.
  • Experience in a community mental health team an advantage.
  • Drivers licence
  • Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).
