Intensive Community Treatment Clinician – Aged (SW3)

Your new team

  • We are a dynamic, diverse multidisciplinary team who strive to make life better for the people we work with, their families and carers
  • We work primarily with adults aged over 65, in an acute phase of mental illness, who need support in their recovery through an outreach model of care and with a recovery centred focus.
  • We enjoy a collaborative and respectful working environment where clinicians are encouraged to share clinical judgements and participate actively in treatment planning with the medical staff
  • We offer a wide range of therapeutic interventions to people including supportive anxiety management, family work, medication administration, safety planning

Your new role

  • Deliver evidence based treatment and interventions that support older people with complex mental health and/or ageing related needs, and their families and carers
  • Participate in clinical decision making
  • Communicate effectively with consumers, families/carers, colleagues and other service providers

About you

  • Experience as a mental health clinician in either a community or inpatient setting
  • You are an effective communicator with consumers, families/carers, colleagues and other service providers
  • Accurately prepare documentation in a timely manner
  • Work well and autonomously within a supportive and dedicated small team
  • You have a degree in Social Work and eligibility for membership of the Australian Association of Social Workers

All appointments are made subject to a satisfactory Criminal History Record Check and a Working with Children Check. Successful applicants will be required to provide evidence of an immunisation assessment and COVID-19 Vaccination prior to commencement.

Selection Criteria Desirable: An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.
Desirable: At least 7 years post graduate experience in the assessment and treatment of persons with a mental health diagnosis. Demonstrated skill and experience in community based management of services, assessment and treatment of older adults with a mental illness and dementia.
Desirable: Demonstrated ongoing commitment to professional development.
Desirable: Excellent organisational and time management skills. Current knowledge of effective evidence-based interventions appropriate to Mental Health Diagnoses. Confident to work autonomously, problem solve and take initiative. Highly developed communication and interpersonal skills.
Desirable: Accurately prepare documentation in a timely manner. Experience of working collaboratively with consumers, carers, aged care facilities and other organisations Demonstrate ability to work as part of a team A current driver’s licence is fundamental to the position
