About your team

 

The RMH Kidney Care Service encompasses all aspects of the care of patients with kidney disease – Nephrology, Dialysis, Transplantation and Nephrology Surgery.

 

The Department delivers a continuum of care for people with chronic kidney disease (CKD) and kidney failure including CKD care, transplantation, dialysis, conservative care and end of life care.

 

Our inpatient, outpatient, outreach and remote services are designed to be appropriate and timely to meet a broad range of patient needs. Services are developed in consultation and collaboration with our patients, families, partner health services, primary health care and community services. The RMH Kidney Care service includes 1 acute dialysis service (6W), 1 home training unit (RPC), 2 metropolitan dialysis units (Essendon and Coburg), and 23 regional dialysis units.

 

Your new role

 

We are looking for a passionate and dedicated individual to join our Nephrology team. Our Coburg Dialysis Unit has a full time permanent position as a Clinical Case Manager (CCM) Grade 4B.

 

Provide ongoing assessment and monitoring of patient’s haemodialysis and healthcare needs

Respond to specific requirements, and communicating issues to appropriate personnel. 

Work as part of a diverse multi-disciplinary team in the Department of Nephrology 

 

Your contribution: 

  • Registered Nurse Division 1
  • Extensive haemodialysis experience       
  • Excellent communication and interpersonal skills
  • Effective problem solving skills
  • Team focused with a desire to care for people
  • Renal qualifications are desirable
