  • Part Time Fixed Term for 12 months working 64 hours per fortnight

The Cognitive Dementia and Memory Service (CDAMS) aims to assist in diagnosing and providing support to people who are experiencing changes in their thinking, behaviour or memory. The CDAMS multi-disciplinary team provides clinical diagnosis, information on appropriate management options and direction when planning for the future. The team is committed to working collaboratively to provide expert diagnosis and advice regarding strategies and supports available. CDAMS is a Specialist Clinic within the Sub-acute Ambulatory Care Service (SACS).


Your new role:

  • Home based functional assessment of cognition that contributes to the diagnostic picture.
  • Provide cognitive strategies and link patients to resources and community supports.
  • Contribute to the CDAMS care plan at the multi-disciplinary meeting.
  • Provide information and support to patients and families regarding diagnosis, management and supports.

About you:

  • Bachelor of Occupational Therapy (or equivalent) and professional registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • A current Victorian driver's licence.
  • Demonstrated clinical competence in the assessment and management of clients with dementia.
  • Display sound organisational skills through efficient time management, professional responsibility and accountability.
