  • Care Coordinator - Hospital Admission Risk Program
  • Part Time Fixed term until 30/06/2023 working 53.2 hours per fortnight
  • Based at Royal Park Campus

The HARP service aims to improve clients’ health outcomes and reduce the risk of unnecessary representations to the hospital and the emergency department.  Services are delivered to the clients/carers in their homes, community health services or outpatient settings; through face-to-face, telephone or telehealth modalities. 


Your New role

  • Triaging and identify recently discharged clients who are at high risk of re-presenting to hospital and engage via telehealth / phone to develop appropriate plans of care to address their health, psycho-social issues.
  • Ensure clients receive adequate health education / information in an appropriate and accessible format to support their post discharge care needs.
  • Actively support clients to make informed decisions about their treatment options and ongoing care needs, including appropriate medical follow up as required.
  • Contributing to data analysis and evaluate, to ensure changes are implemented effectively

About you:

  • Experience in complex needs assessment, care planning and coordination for clients with chronic disease and complex psychosocial needs
  • Understand factors contributing to hospital admission in clients with complex needs
  • High level knowledge of Community Services
  • Demonstrated ability to be flexible, energetic, innovative, and work cooperatively in a multidisciplinary team as well as autonomously
  • Competent IT user, with well-developed written and verbal communication skills
