Are you looking to provide patient centred care as part of an award-winning dynamic team?

Then we are looking for you.  We are looking for an Occupational Therapist with excellent communication skills who is looking to be part of a treating team that works with and for patients who are transitioning through their health care journey.

The role of the Occupational Therapist is to provide patient health promotion; assist with provision of therapeutic services to assist patients in maximising their functional abilities; assessment and direction of therapy to support the patient; source and assist with access to appropriate equipment to support the patient on the ward and to aid in discharge planning.  You are also required to support collaborative care within the team for the patient.

Transition Care is a 35 bed inpatient unit.  29 beds are Transition Care beds and these patients are cared for on the unit whilst awaiting residential care placement or are in the process of making decisions related to long term care needs and how that will look for the patients and their families.  Our patients are generally older adults and are recovering from an acute admission. 


6 beds at the unit are GEM Restorative and allow for older adults who may be recovering from surgery or awaiting finalisation of plans for future care and assessing whether further rehabilitation is required to promote the best long term care requirements for the patients and their families.   


The Transition Care Program provides support to patients and families whilst reviewing and assisting with decisions regarding long term care needs.

All appointments are made subject to a satisfactory Criminal History Record Check and where required, a Working With Children Check. Successful applicants will be required to provide evidence of an immunisation assessment prior to commencement
