Community Psychiatric Nurse

Part Time Fixed Term until 30/06/2017, 32 hours per fortnight

 

Led by CoHealth, Homeless Outreach Mental Health & Housing Service (HOMHS) is a consortium of health services with Inner West AMHS mental health clinicians, CoHealth staff, a Homeground housing officer and a McAuley Community Services for Women worker to provide expertise in mental health to adults with severe and enduring mental health issues and recurrent homelessness. This team will operate as an assertive outreach team using a Collaborative Recovery Model to deliver care coordination and case management to eligible consumers. The initiative aims to reduce homelessness and its devastating health, social and economic impact on people with severe and enduring mental illness.

 

The HOMHS team will be based at Central City Community Health Service and is funded through 2017.

 

We are looking for:

EXPERIENCE - A Community Psychiatric Nurse with at least 2 years experience in the assessment, delivery of treatment and provision of targeted interventions and support to consumers with complex mental health related needs and their families/carers

 

COMMUNICATION - Well developed skills in writing and an ability to promptly prepare documentation and the ability to communicate effectively with consumers, families/carers, colleagues and other service providers

 

ADAPTABILITY – Know when to change priorities to meet the needs of the consumer or team, adjust quickly to new tasks, be flexible and remain positive in a changing environment

 

INITIATIVE – Be proactive, look for opportunities for self-development and improving service delivery. Ability to problem-solve without being asked

 

COLLABORATION - Demonstrate behaviours that support a recovery approach to care. Promote a friendly, respectful and supportive environment within the Community Team and across the AMHS

 

ACCOUNTABILITY – Good organisational and time management skills

 

INTEGRITY - Demonstrate behaviours reflective of Melbourne Health’s values

 

Registration with Australian Health Practitioner Regulation Agency

A multi-disciplinary workforce of 1,800 staff provides services through four Area Adult Mental Health Services (North West, Mid West, Inner West and Northern), an Aged Persons’ Mental Health Program and a Youth Mental Health Service – ORYGEN Youth Health. NWMH is a clinical service of Melbourne Health and operates in partnership with Northern Health and Western Health.

Selection Criteria Desirable: A current Victorian Driver’s Licence, and ongoing ability to use this form of transport.
Desirable: 2+ years’ experience in the assessment, diagnosis, and treatment of severe mental illness and psychiatric disability
Desirable: Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).
