  • FIXED TERM POSITION until 29th June 2018

We are seeking a motivated individual who is able to influence change within an adult acute mental health setting and wprk alongside and promote the consumer and carer peer support workforce.


The Victorian Department of Health and Human Services (DHHS) has provided program funding for the expansion of post discharge support by peer workers. The purpose of this role is to implement and provide Peer support /clinical support to the consumer and carer discharge peer support workforce.


This position requires a clear and readily articulated understanding of the values and practices of the consumer/carer peer support model including roles and responsibilities, and to utilise the principles of peer support in order to achieve the following:

Key Responsibilities:

  • Assist in the identification of consumers, and their family/carer/support people, admitted to the acute mental health inpatient unit who are at risk of readmission after discharge (with the emphasis on preventing re-admissions within 28 days of discharge);
  • Interface with the inpatient unit discharge coordinator and develop pathways for referrals for those consumers & or carers/families identified as requiring expanded post discharge support
  • Provide clarification and direction to in-reach and outreach follow up to identified consumers/families for follow up
  • Contribute to effective discharge planning including oversight of the  peer workers consumer/carer/family contacts both during and after the inpatient stay
  • Assist the peer support workforce to focus on their own well-being through supervision and co reflection activities
  • Support the inclusion of carers/nominated persons in the development of consumers’ recovery and wellness plans; working collaboratively with the discharge coordinator to develop a process pathway for this to occur
  • Conduct and support relevant local orientation and training to the discharge peer support workers and team.

Essential:

  • Excellent communication skills will be required for the formation of strong relationships with the   general health care sector, specialist mental health services and other relevant agencies.
  • Appropriate qualifications, and a commitment to the provision of a client-centred service, are essential.
