Melbourne Health has the largest comprehensive renal service in Victoria and provides a state-wide quaternary service across the chronic kidney disease continuum, with a strong commitment to improvement through research and innovation. The dialysis service provides expertise, clinical governance and support to inpatient, acute, home dialysis, metropolitan satellites and 23 regional satellite services throughout Victoria.

 

The Opportunity


The Royal Melbourne Hospital (RMH) Kidney Care Home Dialysis Service (HDS) is seeking a motivated, self-directed Registered Nurse to educate, support and promote independence for their home dialysis patients.

 

The successful candidate will be well supported by a multi-disciplinary team and their key accountabilities include:

  • Provision of education and support for home haemodialysis and peritoneal dialysis patients and care providers.
  • Participation in after- hours support for home dialysis patients.
  • Participation in the patient care model for Home Dialysis embracing rotation through all roles.
  • Monitoring & assessing patients in the local community including visits to the home and liaise with outpatient clinics.
  • Liaison with key stakeholders regarding patient care including departments within Melbourne Health, regional and rural hospitals, regional physicians and community groups as needed.
  • Developing research and quality activities that measure and contribute to improvements in patient care.

The Candidate


Essential to the position are:

  • Formal Nursing Qualifications with AHPRA
  • Recent renal experience
  • Knowledge of clinical assessment skills
  • Good interpersonal skills
  • Computer literacy – Microsoft Office
  • Current Victorian drivers license
  • Ability to work autonomously with efficient time management skills
  • Desirable:
    • Demonstrated experience in patient education programs
    • Demonstrated ability to participate in and support change
