Team Leader – Homeless Outreach Mental Health (HOMHS)

  • Part-time 0.8EFT, fixed term until 30 June 2021 - SW3, OT3, P3, RPN4

Your new team

 

HOMHS provides support to people living in the Melbourne CBD and council of Moonee Valley who experience severe and enduring mental illness and have a history of repeated homelessness. 

 

  • HOMHS is a cohealth led consortium comprising of cohealth, Inner West Area Mental Health Service and Launch Housing.
  • HOMHS work closely with other Inner West services, in a particular Community Team 1 who hold the homeless portfolio.
  • We are passionate about working with people with mental illness experiencing homelessness.
  • We work within a multidisciplinary team and with community-based organisations.

Your success in this role, will be in your ability to

  • Lead and role model the delivery of evidence-based treatment and interventions, ensuring the provision of high quality assertive outreach and care coordination of services to adults with mental health issues and homelessness.
  • Contribute to team functions including coordination and allocation of referrals, new patient assessments, intake and rostered activities.
  • Be responsible for developing partnerships and collaboration between agencies and across the mental health and homelessness sector.

Your Contribution

  • You must be qualified as either a Registered Psychiatric Nurse, Occupational Therapist, Social Worker or Psychologist.
  • Contribute to a positive culture, demonstrate partnership and collaboration within the team, across consortium agencies and other key stakeholders.
  • Demonstrate extensive experience in the provision of specialist services in public mental health settings with a strong focus on local communities and inter-agency specific knowledge. 
  • Demonstrate recovery-oriented practice.
