Nephrology Patient Flow Coordinator 

  • Fixed Term Position with potential to become ongoing
  • Work as part of the multidisciplinary Nephrology team
  • Play an integral role in patient care from admission to discharge
  • Assist in safe and timely discharge planning based on person centred care

We are looking for a passionate and dedicated individual to join our Nephrology team. This is a Full Time, Fixed Term position, working 80 hours per fortnight, based at RMH Parkville Campus, with the view to be ongoing.

Your Opportunity:

  • Coordinate discharge planning and care that meets the needs of our Nephrology patients, their families and carers
  • Work with the multi-disciplinary team to support the coordination of assessment and monitoring of a comprehensive plan of care from admission to discharge
  • Liaise with post-acute services, referring acute hospitals and allied health to facilitate safe and timely discharge planning
  • The role is four, 10 hour shifts on Mondays, Tuesdays, Thursdays and Fridays.


About You:

  • Current registration with AHPRA.
  • Extensive clinical experience and expertise in Nephrology
  • Good operational understanding of health services processes and metrics
  • Experience in a management / leadership role, such as ANUM or equivalent desirable
