Your success in this role, will be in your ability to

  • Undertake strengths and holistic needs based assessments for older people accessing supports at home
  • Participate in implementing a new team of Home Support Assessors
  • Participate in interdisciplinary supervision

Your Contribution

  • Qualification in Allied Health or Registered Nurse
  • Professional registration according to discipline (AHPRA)
  • Experience or understanding of community & aged care
  • Commitment to providing a high level of care
  • Proactive approach to knowledge development

Selection Criteria Essential: Recognised Allied health degree or equivalent and associated professional registration (AHPRA)
Essential: Person centred verbal, non-verbal and written communication skills
Essential: Proactive approach to knowledge development
Essential: Clinical skills in assessment, care planning and discharge planning
Essential: Driver's licence
Desirable: Experience in working in a wellness and reablement approach to assessments
Desirable: Experience undertaking home based holistic assessments.
Essential: Ability to engage with older consumers from diverse backgrounds and their carers
Desirable: Strong IT skills and experience in mobile assessments
